Sample Activity Progress Notes

Writing activity progress notes can be time consuming but it is an important aspect of documentation. F-tag 248 clearly states that there must be a method in which each resident's program of activities is monitored. There are many ways to complete progress notes. Below are two examples of a quarterly progress note.

Since her admission on 10/9/01, Mrs. X has pursued an active leisure lifestyle. She is alert and oriented to all realms and makes her needs known. She is sociable with both staff and peers and initiates conversations. Her family members are very supportive and visit weekly, providing additional reading materials and Mrs. X's favorite foods. Mrs. X is an active participant in daily recreational activities both on and off the unit including exercise programs, adapted physical games (bowling, basketball, ball games), word games, bingo, trivia, discussion, Beauty groups, music programs, cooking and crafts. Mrs. X also joined the Resident Council and has attended 1x this quarter. She has been invited to attend the Young Adult Group, however has declined. She enjoys going to the greenhouse for horticultural therapy sessions and the off unit special events. Although a practicing Roman Catholic, Mrs. X states she finds much strength in attending chapel services of all faiths. Mrs. X enjoys a variety of independent activities including: talking on the phone in her room, listening to music on her Walkman (country and 50-80's music), reading large print books, crossword puzzles and watching movies in her room. Recreation Staff provides movies, crossword puzzles and large print reading materials. Mrs. X has episodes of tearfulness and often dwells on her illness. Recreation staff encourages her to pray, listen to favorite music, or find solace in the greenhouse if Mrs. X does not express interest in joining the group activity. Recreation staff will continue to invite Mrs. X to daily recreational activities and provide supplies for independent pursuits. In addition, Mrs. X will be encouraged to attend the Young Adult Group to spend time with residents of her age group for additional support and companionship. Mrs. P remains alert with confusion and forgetfulness. She is oriented to self and her speech is limited. She responds to simple direct questions by nodding her head or mumbling. Mrs. P is OOB daily in a recliner chair and spends much of the day in the dayroom where she is engaged in recreational activities including Snoezelen, Five Alive, Sensory and Movement, Music Therapy and adapted reminiscing groups. R/t to poor cognitive status, Mrs. P requires extensive assistance (i.e. hand over hand, physical/verbal cueing) to participate in leisure programs. R/t contractures of both arms she is provided with U/E and L/E PROM during adapted physical games and exercise programs to maintain physical functioning. During sensory programs, a variety of Recreational techniques are utilized to elicit responses to stimuli including: Aromatherapy for olfactory stimulation; hand massages, textured objects and props (yarn) for tactile stimulation; pictures of interest (cats) fiber optic lights and bubble towers for visual stimulation; ice cream, pudding, flavored lip balms etc. for gustatory stimulation; and music (Italian and singalong) as well as nature sounds, for auditory stimulation. Mrs. P responds to sensory stimulation by tracking objects, vocalizing, smiling, laughing, humming, and tapping feet and hands. She especially enjoys Italian music and the song "O Solo Mio". She also participates in discussion and reminiscence groups when asked closed-ended questions. Recreation staff provides one to one sensory programs PRN. Family visits are irregular. Recreation staff will continue to provide sensory stimulation to enhance responses to external stimuli and maintain quality of life. History of Baking

Baking has been many cultures' favorite technique for creating snacks, desserts, and accompaniments to meals for many years. Now, it is very well-known as the method for creating sweets and all sorts of wondrous mouthwatering pastries. In ancient history, the first evidence of baking occurred when humans took wild grass grains, soaked it in water, and mixed everything together, mashing it into a kind of broth-like paste. Then, the paste was cooked by pouring it onto a flat, hot rock, resulting in a bread-like substance. Later, this paste was roasted on hot embers, which made bread-making easier, as it could now be made anytime fire was created. Around 2500 B.C., records show that the Egyptians already had bread, and may have actually learned the process from the Babylonians. The Greek Aristophanes, around 400 B.C., also recorded information that showed that tortes with patterns and honey flans existed in Greek cuisine. Dispyrus was also created by the Greeks around that time and widely popular; was a donut-like bread made from flour and honey and shaped in a ring; soaked in wine, it was eaten when hot.

In the Roman Empire, baking flourished widely. In about 300 B.C., the pastry cook became an occupation for Romans (known as the pastillarium). This became a very highly respected profession because pastries were considered decadent, and Romans loved festivity and celebration. Thus, pastries were often cooked especially for large banquets, and any pastry cook who could invent new types of tasty treats, unseen at any other banquet, was highly prized. Around 1 A.D., there were more than three hundred pastry chefs in Rome alone, and Cato wrote about how they created all sorts of diverse foods, and flourished because of those foods. Cato speaks of an enormous amount of breads; included amongst these are the libum (sacrificial cakes made with flour), placenta (groats and cress), spira (our modern day flour pretzels), scibilata (tortes), savaillum (sweet cake), and globus apherica (fritters). A great selection of these, with many different variations, different ingredients, and varied patterns, were often found at banquets and dining halls. To bake bread, the Romans used an oven with its own chimney and had grain mills to grind grain into flour.

Eventually, because of Rome, the art of baking became widely known throughout Europe, and eventually spread to the eastern parts of Asia. Bakers often baked goods at home and then sold them in the streets-children loved their goods. In fact, this scene was so common that Rembrandt illustrated a work that depicted a pastry chef selling pancakes in the streets of Germany, and young children surrounding him, clamoring to get a sample. In London, pastry chef sold their goods in handcarts, which were very convenient shops on wheels. This way, they developed a system of "delivery" baked goods to people's households, and the demand for baked goods increased greatly as a result. Finally, in Paris, the first open-air café of baked goods was developed, and baking became an established art throughout the entire world.

Many people do not even know that they are being abused verbally or emotionally. I was one of those people. I thought that everyone was being treated like I was. I thought that everyone was walking on eggshells afraid of how their spouses were going to react.

Abusers come in all shapes and sizes. They can be a male or a female. They can be a parent or a spouse. I am using my personal example of my ex-husband of twenty one years of marriage.

There were good days when we could sit and laugh and talk about things that were important to both of us. When those days would come, I would relax and enjoy myself but I was always guarded. When was the next blow-up? How would I react? What would he say? Where would the children be? How long could I hold my tears in so my children wouldn't notice that I was crying again?

I can do anything. You are nothing to me. I am pushing back. You are a chicken. You can't even fight me. Wait and see now what is going to happen to you"

A way to come up with a good eulogy is to just find tips and suggestions that will help you write your own eulogy.

1. Collect your material. Gather facts about the person - age, important dates, favorite places, and so on. 2. Choose a theme. What gives a eulogy unity is a theme. A theme will help you and your listeners distinguish the patterns of the life of the deceased. 3. Organize your speech. You may want to use one index card per significant memory. In this way, you can easily filter which memories you want to include in your eulogy speech.

4. Write your draft. Write your first draft using the information you collected and the memories you wrote in index cards. You can go through each index card and choose the best memories to include in your speech. Use a word processor so that editing your speech will be easier afterwards. Put your focus on the beginning and ending of your speech. Do not try hard to be formal; use a conversational tone. Include humor if you wish to. It is an effective way to ease the pain and tension a little if you are able to laugh at things. Include appreciation and gratitude not only to the deceased, but also to the people he or she has shared life with.

5. Rehearse your speech. If you are not used to delivering speeches, aside from the free eulogy samples you took, you may also want to borrow materials that will help you do the public speaking trick. Rehearse your speech to find awkward lines; this is your chance to do some editing. Remember to calm down and be natural. Nobody is against you so you do not have to fear anything.

Calculate the Coefficient of Variation in Excel

MS Excel is a powerful tool for crunching statistics. You may want to learn how to calculate the coefficient of variation, which is simply the standard deviation divided by the mean. Here how:

1. First, isolate a range of cells who coefficient of variation you want to know. For this example, let's assume that the data who coefficient of variation you want to calculate is in cells A2:A20.

2. Calculate the standard deviation of this data, the formula for which in this case is: =STDEV(A2:A20). Place that formula in cell C1.

3. Now, calculate the mean (average) for the same range by using this formula: =AVERAGE(A2:A20). Place that formula in cell C2.

4. To get the coefficient of variation, type this formula into cell C3: =C1/C2.

Another of the most useful statistics functions that Excel can crunch is that of variance. The variance value tells you, in effect, how uniform a sample of values (either a sample or an entire population) is as a whole. The formula for variance expressed in textual form is:

The sum of the difference between each value in the sample and the mean of all values in the sample (squared), divided by one less than the sample size.

The good news is: you don't need to memorize this formula. Rather, Excel will figure it out for you. Note: some people say they want to "calculate the coefficient of variation" in Excel. In reality, what they are saying is that they want to calculate the variance in Excel.

There are actually four different functions in Excel used to calculate variance. The formula you should choose depends upon:

a. whether the data you have access to represents a sample or the entire population

b. whether you want Excel to include any logical values (TRUE/FALSE values) and any text that can be interpreted as a value that it finds in the range you specify

Here is how to choose which of the four functions for calculating variance in Excel:

1. Use =VAR() when you are basing your calculation on a sample and you want Excel to ignore logical (TRUE/FALSE) values and text.

2. Use =VARA() when you are basing your calculation on a sample and you want Excel to treat logical values as TRUE=1, FALSE=0 and to interpret text values as numbers. Note: this formula will cause an error if the range of cells you select contains text that cannot be interpreted as a number.

3. Use =VARP()when you are basing your calculation on the entire population and you want Excel to ignore logical (TRUE/FALSE) values and text.

4. Use =VARPA()when you are basing your calculation on the entire population and you want Excel to treat logical values as TRUE=1, FALSE=0 and to interpret text values as numbers. Note: this formula will cause an error if the range of cells you select contains text that cannot be interpreted as a number.

How does the variance formula actually look in action? Here's an example. Let's say that the set of data whose variance you want to calculate is in cells A2 to A15. And, assume you have determined that you want to use the VAR version of the formula (see above). Here is the formula you should use in this case:

=VAR(A2:15)

That's it! Excel will return the variance figure you are looking for.

So using the Auto VIN Decoder in the above example, this car is a Ford or an Eagle..since Eagle is no longer made, it is most likely a Ford.

The eighth character is the engine type.

The tenth character represents the year of the car. Pay close attention to this one:

B = 1981 F = 1985 K = 1989 P = 1993 V = 1997 1 = 2001

The eleventh character indicates the assembly plant.

In the above example, the C indicates Ontario, Canada

Characters 12-17 represent the vehicle's unique fingerprint. It is these six digits which make every single vehicle in the world different.

So using the Auto VIN Decoder one last time, the Vehicle Identification Number: 2FTRX18W1XCA01212 represents a 1999 Ford F150 Pickup 4WD Super Cab manufactured in Ontario, Canada with hydraulic brakes and a 4.6-liter V-8 engine.
